What happened?
On or around May 22, 2025, ApolloMD identified unusual activity within its IT network. Following this discovery, the company initiated an investigation, which revealed that an unauthorized third party gained access to its network between May 22 and May 23, 2025. During that period, the unauthorized actor may have viewed or obtained certain files containing personal information related to patients treated by ApolloMD’s affiliated physicians and practices.
The information potentially involved includes names, dates of birth, addresses, Social Security numbers, diagnosis information, provider names, dates of service, treatment information, and health insurance information. Up to 626,540 individuals have been affected by this data breach.
